Foundation Package
The Foundation Package provides a comprehensive assessment and individualized care plan to help families better understand their loved one's needs and identify practical next steps.
This package includes:
• Two intake questionnaires, one for the person living with dementia and one for their primary caregiver, to help me gain a thorough understanding of your situation before we begin working together.
• A comprehensive dementia care assessment conducted in your home or another comfortable setting. This whole-person assessment considers cognitive function, physical health, daily functioning, emotional well-being, social needs, personal history, preferences, strengths, and areas of concern.
• A personalized dementia care plan that includes:
Assessment findings
Individualized goals of care
Recommended interventions and support strategies
Safety recommendations
Emergency planning
Resource recommendations tailored to your family's needs
Practical solutions for common caregiving challenges
• Educational materials and guidance related to topics such as:
Understanding dementia and disease progression
Dementia-related behaviors
Home safety and wandering prevention
Future planning considerations
Caregiver stress and burnout
Bathing, dressing, communication, and other daily care challenges
• A caregiver organization binder with templates and tools for tracking medical information, medications, appointments, legal documents, emergency contacts, and other important records.
• Follow-up support at one week and one month to answer questions, review recommendations, and discuss any changes or concerns.
Guidance Package
The Guidance Package includes everything in the Foundation Package, along with ongoing support and monitoring during the first three months following the assessment.
In addition to all Foundation Package services, this package includes:
• Weekly check-ins for three months to provide guidance, answer questions, and address emerging concerns.
• A reassessment and updated care plan at three months, or sooner if a significant change in health, cognition, behavior, or functioning occurs.
• Advocacy support at one physician or specialist appointment, including note-taking, question preparation, and assistance understanding recommendations and next steps.
Comprehensive Support Package
The Comprehensive Support Package provides the highest level of ongoing support for families managing complex dementia care needs.
This package includes everything in the Foundation and Guidance Packages, plus:
• Weekly check-ins for six months.
• Reassessments and updated care plans at both three and six months, or sooner if significant changes occur.
• Advocacy support at up to three physician or specialist appointments.
This package is ideal for families who would benefit from consistent support, ongoing care planning, and a trusted partner to help navigate the many changes that can occur throughout the dementia journey.
