Placement & Transition Support Package

A comprehensive assessment to evaluate your loved one's health, cognitive and functional abilities, care needs, preferences, goals, and circumstances. This assessment helps determine the most appropriate level of care and support.

Personalized Facility Recommendations
A curated list of 3–5 facilities selected based on your loved one's individual needs, budget, preferred location, lifestyle preferences, and care requirements.

Tour Preparation and Accompaniment
Guidance in developing questions to ask during facility tours and identifying important factors to evaluate when comparing options. I can also accompany you on facility tours to provide support, take notes, and help you make informed decisions.

Emotional Support and Guidance
Compassionate support throughout the decision-making process as you navigate the many emotions and challenges that often accompany a move to a higher level of care.

Transition Coordination
Support before, during, and after the move, including:

  • Coordinating movers and other services

  • Identifying supplies, equipment, and items needed for the new living environment

  • Assisting with paperwork and record organization

  • Communicating with facility staff to promote a smooth transition

  • Creating a personalized transition plan

Follow-Up Support
